In accordance with the policy, this form will ask you to specify whether the appointment you request is a Graduate Faculty appointment or an Affiliated Graduate Faculty appointment. You will also be able to choose "Other Program Faculty:" this means we will include the person in the lists of your faculty, but they will have none of the responsibilities or privileges described in the LGS policy.
