Program Transfer Request

    This form is intended for students who want to transfer from one degree program into another degree program within the LGS.

    Fields marked with * are required.

    If you are considering transferring between programs in LGS, it is best to have a conversation with the LGS Assistant Dean of Student Affairs early on in the process of deliberation, and certainly before you submit this form.

    This form will be routed for approval to the Program Director / Director of Graduate Studies in both your current program and your intended new program.  The program transfer must also be approved by the your Division Director (if applicable) and by Dean of the Laney Graduate School.  A transfer may have implications for your funding situation and for the timeline of your academic progress, and both will be reviewed as part of this process.

I attest that I will be able to reach candidacy no later than September 15 of the start of my fourth year, and that I will be able to comply with all other applicable standards of satisfactory progress.
