    This form is intended for doctoral students who are leaving their programs can discuss with their Director of Graduate Studies the option to leave with a terminal master's degree.

    Doctoral students who discontinue their PhD graduate study may be eligible for a terminal masters degree. Doctoral students who are leaving their programs should discuss the option to leave with a terminal master's degree with their Director of Graduate Studies of Program Director.  If you submit this form, your DGS / PD will be asked to support your request.

    IMPORTANT: If you are completing this form after the degree completion deadline, stop and submit this form at the start of the next term.

    Before submitting this form, you must develop a written plan and timeline for degree completion with your faculty advisor and submit it with this form.

    You should not submit this form before consulting with your program DGS.Your DGS must consult with your faculty advisor of any possible changes to your funding and student health insurance. If completion of the terminal master’s degree extends beyond the term approved for the master's degree, you may receive a tuition scholarship and may not receive a stipend or student health insurance subsidy. Students will be responsible for the EUSHIP premium and remain responsible for all student fees. Any supporting materials regarding funding should be submitted with this form.

    Approval and Confirmation
    When you submit this form, we will contact your DGS for their approval. It is important that you confer with your program first, so they are aware of and support your request. If your program does not support it, then we will deny this request. If your DGS approves your form, it will be reviewed by the LGS Finance Team who will confirm if there will be changes to your funding and health insurance. Once confirmed, you will receive an email to schedule a meeting with a member of the LGS Student Affairs team for a final review of your academic plan.
